As a Physio, One of the most common I've had rolling around inside my head for decades is that you should avoid "open chain" exercises - like leg extensions - because they might put too much stress on your new graft.
This belief has been around since the 1980s, and it has shaped how many physiotherapists and surgeons approach ACL rehabilitation. For years, the standard approach was to focus almost exclusively on "closed chain" exercises like squats, leg presses, and step-ups, while avoiding exercises where the knee moves without the foot in contact with the ground.
But is this approach actually supported by the evidence? A new systematic review and meta-analysis published in Physical Therapy in Sport has examined this question in detail. The researchers analysed 14 studies involving 729 participants to determine whether open kinetic chain exercises are safe and beneficial in the early weeks after ACL reconstruction. 2. The Traditional Concerns: Why Were OKC Exercises Avoided?

This creates shear forces that might directly pull on the graft, potentially leading to graft failure or increased knee laxity.
Because of this concern, the standard approach in many rehabilitation programs was to avoid OKC exercises entirely, especially in the early weeks after surgery. The focus was instead on CKC exercises, which were believed to be safer because:
- The compressive forces through the joint provide more stability
- Multiple muscles work together, reducing isolated stress on the graft
- The foot being fixed means the forces are distributed differently
However, the researchers behind this new study point out that these concerns "may be outdated, as the stress imposed by OKC exercises may be equal to that imposed by daily activities or other strengthening exercises".
3. What the New Research Shows
The systematic review included 14 studies with 729 participants and analysed 31 different outcomes. The researchers found that OKC exercises had beneficial effects on:
- Patient-reported outcomes (PROMs): People who did OKC exercises reported better function and quality of life, as measured by scores like the IKDC.

A separate study found that introducing OKC hamstring exercises as early as 3 weeks after surgery was safe and promoted clinically meaningful improvements in strength and endurance. However, the researchers of the main review caution that starting before 4 weeks may increase laxity measurements, even if it does not cause graft failure.

A scoping review found that clinical criteria like achieving 100 degrees of knee flexion, minimal swelling, and pain below 2-3/10 were also used to guide progression.
