If you have thoracic outlet syndrome, RSI in your hands and fingers, tennis elbow, or any other issues further down your arm, it is worth considering that the nerves supplying those tissues exit from a space that is in part created by the first rib.

In this article, I want to show you a simple but effective mobilisation exercise that can help free up first rib dysfunction and potentially provide relief for a wide range of upper body complaints.

1. Why the First Rib Matters

The first rib is located at the junction between your neck and your upper back. It forms part of the thoracic outlet – the space through which nerves and blood vessels travel from your neck down into your arm.

When the first rib becomes stiff or restricted, it can contribute to a wide range of issues:

  • Neck pain and stiffness – the first rib attaches to the spine and can influence neck mechanics from below.
  • Shoulder problems – restricted rib movement can alter shoulder blade positioning and mechanics as it sits underneath.
  • Thoracic outlet syndrome – compression of nerves and blood vessels in the thoracic outlet can cause pain, numbness, and tingling down the arm.
  • RSI and tennis elbow – nerve irritation originating from the first rib area can refer symptoms and dysfunction further down the arm.
  • Upper back stiffness – the first rib is part of the upper rib cage and can affect overall thoracic mobility.

2. How to Perform the First Rib Mobilisation

Step-by-Step Guide

  1. Find the landmark: Locate the big, thick bump at the base of your neck – the junction between your neck and upper back.
  2. Position the ball: Place a lacrosse ball or tennis ball right on the bump, then roll it just off to the side onto the fleshy part.
  3. Apply pressure: Lie down on top of the ball, letting your body weight provide gentle, sustained pressure.
  4. Find the restriction: Move the ball up and down slightly, searching for spots that feel stiff, tight, thick, or restricted – not just tender.
  5. Hold and breathe: Stay on a tight spot for 30-60 seconds, breathing deeply to help your nervous system relax.
  6. Work the area: Move the ball up and down to address the first rib, second rib, and surrounding spinal joints.
  7. Test arm movement: With your arm straight, check how far you can reach – you should notice improvement as the joint mobilises.

As you become more comfortable, you can gently oscillate in and out of your range to help mobilise the joint more effectively.

3. What to Look For

A common mistake when using a ball for self-mobilisation is focusing only on tender spots. While tenderness should be respected, the most important areas to address are those that feel restricted.

Key Point: You could have one side that feels really tender and symptomatic, but if the other side is very stiff, tight, thick, and restricted, this is the side that you may actually need to focus on to make both sides function and feel better.

Here is what to look for:

  • Stiffness: Does the tissue feel harder or denser than the other side?
  • Restriction: Does it feel like the tissue is resisting the pressure of the ball?
  • Thickness: Is there a sense of more tissue density than on the other side?
  • Asymmetry: Compare left and right – which side feels more restricted?

Once you have identified a restricted spot, apply sustained pressure for 30-60 seconds while breathing deeply. You can also gently oscillate in and out of the pressure to help mobilise the joint.

4. The Test-Retest Approach

"I don't want you to have to trust me. Make sure that you can see the impact that mobilising the first rib has on your symptoms and how your body feels and functions."

The best way to know if this technique is working for you is to use the test-retest approach:

  1. Before mobilising: Test your arm range of motion (reach as far as you can with your arm straight), neck rotation, or any specific movement that reproduces your symptoms.
  2. Perform the mobilisation: Spend 2-3 minutes working on the first rib and surrounding areas.
  3. Immediately re-test: Repeat the same movement and notice what has changed.

What you should expect:

  • Your arm may reach further
  • Your neck may move more freely
  • Shoulder movement may improve
  • You may breathe more easily
  • Symptoms in your arm (tingling, numbness, pain) may reduce

5. Addressing the Root Cause

If you find that your first rib is stiff and restricted, the next question is: why? Mobilisation can provide temporary relief, but lasting change requires addressing the underlying factors that created the restriction in the first place.

Common Contributing Factors

  • Prolonged slouching: Sitting with a rounded upper back can compress and stiffen the first rib area.
  • Forward head posture: This places additional strain on the upper thoracic and cervical junction.
  • Desk work: Extended periods in a fixed position can lead to adaptive shortening of tissues.
  • Repetitive arm movements: Especially overhead or forward-reaching activities.
  • Breathing patterns: Shallow chest breathing can limit rib mobility.

Once you have identified the restriction and mobilised it, it is worth having a conversation about what postures and shapes you are getting into that load up that same area. Small changes in your everyday habits can make a big difference in preventing the restriction from returning.
